Vehicle Request Form

1. Arrangements must be made to pick up the vehicle key during office hours...Monday-Thursday, 8 am - 5 pm or Friday 9:00 am - 3:00 pm. Key needs to be returned when the vehicle is returned.
2. Primary drivers must be at least 25 years of age or a member of the Seaford Staff. They can't have had more than one ticket in the past 3 years and they can't have ever had a suspended driver's license.
3. Alternate drivers must be at least 21 years of age and have prior approval by the Administrator. The primary driver must be in the vehicle with the alternate driver and is responsible for the vehicle at all times.
4. All passengers on board will wear seat belts (mandatory). Anyone caught not wearing them will cause the person in charge to lose their vehicle privileges.
5. Upon return to church, vehicle must be cleaned up. Trash must picked up and swept out.
6. The vehicle must be filled up with gas before it is returned.
7. Any problems with the vehicle during usage must be reported to the Facility Manager.
8. While vehicle is in your possession, it is your responsibility to take care of it and drive it in a safe and courteous manner.
